Hospital at Home
The Department of Hospital Medicine at Beth Israel Lahey Health / Lahey Hospital and Medical Center is actively seeking a Virtual Hospitalist to work closely with the Medical Director, Clinical Operations Director and Nursing Director of the Hospital at Home program.
The Virtual Hospitalist will work in the Virtual Hospital telemedical command center ( Mission Control ) and ensures provision of high-quality virtual hospital care.
The ideal candidate is passionate about transformative virtual care solutions, innovating clinical best practice, and driving exceptional patient-centered care.
Position Summary :
- Provide high quality, patient centric, cost effective in-patient level of care in the home setting.
- Clinical outcomes are equal to or better than inpatient hospitalization.
- Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center
- Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions.
- Improved patient experience.
- State of the art equipment at Command Center and patients’ home.
- Patients have home infusion, meals / nutrient consults, pharmacy / medical mgmt., mobile imaging, laboratory and DME / oxygen.
- Integrated care with primary care providers, specialists & ancillary departments.
Essential Duties & Responsibilities including but not limited to :
Facilitating the triaging of eligible, appropriate patients referred for a potential admission facilitated by Medically Home guidelines;
obtaining additional diagnostic and specialty consultation when needed
- Providing inpatient level acute care in the home setting in coordination and collaboration with the advanced practice provider, disease related specialist and Mission Control RN
- Providing protocol driven assessment, care coordination and interventions as appropriate informed by Medically Home protocols for diverse patient populations with a focus on Medicine, oncology, and post-operative care
- Exhibiting sound clinical decision making and critical thinking skills
- Perform timely, accurate and thorough documentation of patient’s clinical condition(s).
- Completing the patient referral and providing safe handoffs to the advanced practice provider and Mission Control RN
- Communicating with the patient and family concerning the Medically Home model, incorporating patient and family values and goals in the plan of care
- Navigating patients and families across the continuum of the Medically Home phases of care.
- Navigating barriers for patients and those within the system to accomplish optimal patient experience and outcomes
- Promoting a collaborative relationship and effort between the advanced practice providers, attending physicians, Mission Control RN, and supplier providers
- Serving as a role model for professional practice
Education :
Licensure, Certification, Registration :
- BC / BE Internal Medicine / Family Medicine / Geriatrics / Emergency Medicine
- Licensed in the state of Massachusetts
Experience :
- Experience as a hospitalist or SNF a plus.
- Committed to providing high quality, evidenced based medicine to a diverse patient population.
- Excellent clinical and communication skills.
- Demonstrated ability to work in a team environment.
