The Role You Play
As an AOT (Assisted Outpatient Treatment) Case Manager at Pawnee you will be directly assisting clients as they move through the phases and levels of an AOT program.
You will be responsible for educating your caseload of clients and their families about the symptoms of a mental health diagnosis, as well as medication information and access to community resources.
This will all be done while understanding and engaging with the clients personal history and needs / wants regarding their own treatment.
- The Day-to-DayCreate a treatment plan specific to each client, engaging directly with them in a purposeful, supportive, and helpful way while promoting their own agency and choicesProvide ongoing strengths assessments, activities, and goal development / treatment planning servicesThese will include active participation of the client and any other they wish to include such as extended family membersAssist the client with symptom management in everyday situations, for example at the work site or related to academic progressProvide crisis management, or coordination of emergency services during a mental health crisis situation
- Other Duties as AssignedProvide resources and education to families about mental illnessTransport clients based upon service needsAssist adult consumers with maintaining the benefits they need to successfully continue the rehabilitation process

- RequiredBachelors degree in a human services field or equivalent work experienceReliable transportation, a clean driving record, and automobile liability insuranceExcellent communication skills, both written and verbal, using the English languageBasic computer skills
- PreferredKnowledge of the needs and challenges of adults who experience severe and persistent mental illnessPrior experience in a combined substance use / mental health setting and / or working with the criminal justice systemExperience with Microsoft Office
