Salary :

Care Management includes providing supportive consulting, family relationship management, healthcare crisis management, coordination of healthcare, home safety evaluations, and life care management to clients.

They also serve as account managers for clients receiving home care services and nurse-aides providing their direct care.

Day to Day :

The RN Care Manager is central to the delivery of our premium style of in-home personal care, specifically, by :

• Introducing our services to prospective new clients and conducting initial nurse assessments.
• Selecting and preparing teams of Caregivers to provide hands-on care for clients.
• Developing and nurturing consultative relationships with clients and their families
• Training and encouraging Caregivers as they deliver long-term care.
• Collaborating with our Care Coordination, Recruitment, Finance, and Growth teams.
• Conducting professional geriatric care management consultations (as needed).

Key Responsibilities :

• In-Home Care Assessments :
• Conduct in-home assessments to evaluate healthcare needs of clients.
• Collaborate with the interdisciplinary team to develop personalized care plans.
• Care Management :
• Utilize nursing skills to provide expert care and support to clients in their homes.
• Maintain accurate and up-to-date client records
• Client / Caregiver satisfaction :
• Support and advocate for Clients and Caregiver

Ideal Candidate Profile :

• Experienced RN
• At least 5 years of experience in the nursing field.
• Resourcefulness :
• Knowledgeable about local resources and services related to aging, home care, and senior living options.
• Commitment to Service :
• Passionate about making a positive impact in the lives of older adults and their families.
