We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dynamic team.Position Overview : As a Licensed Practical Nurse (LPN) , you will play a vital role in delivering patient-centered care under the supervision of registered nurses and physicians.

You will be responsible for providing direct patient care, assisting with daily living activities, and ensuring the comfort and safety of our patients.

Key Responsibilities : Administer medications and treatments as prescribed by healthcare providers.Monitor patient health status and report any changes to registered nurses or physicians.

Assist in the development and implementation of individualized care plans.Provide wound care and perform basic clinical procedures.

Educate patients and families on health management and self-care techniques.Maintain accurate and updated patient records.

Collaborate with the healthcare team to ensure continuity of care and patient needs are met.Adhere to all facility policies, procedures, and regulatory requirements.

Qualifications : Current LPN license in CTCompletion of an accredited LPN program.Previous experience in a healthcare setting preferred but not required.

Strong communication and interpersonal skills.Compassionate, patient-centered approach to care.Ability to work effectively in a fast-paced environment.
