LPN Inpatient Case Manager - Hybrid Riverside CA

Primary Responsibilities : + Identify, coordinate and / or provide appropriate levels of care under the direct supervision of an RN or MD+ Perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating).

This includes case management, coordination of care and medical management consulting+ Provide health education, coaching and treatment decision support for membersExpect to travel about 50% of the time to meet with members in the hospital.

Required Qualifications : + High School diploma or GED or equivalent work experience+ Unrestricted LPN or LVN license in the State where the position will be performed+ 3+ years case management or acute care experience+ Basic computer proficiency (Microsoft Word, Outlook and internet)+ Ability to travel 30-50% of the time to local hospitals on a daily basis Preferred Qualifications : + Experience with EMR (Electronic Medical Records)+ Inpatient Hospital Case management experience+ Bilingual - English / Spanish California Residents Only : The hourly range for this role is $19.

47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
