Position Summary
The Director of Case Management is tasked with leading a dynamic team of clinical professionals to ensure exemplary cross-continuum care coordination.
This role involves developing and implementing the case management process, coordinating and supervising the duties of clinical case managers and social workers, and promoting quality patient care outcomes while managing resource utilization effectively.
The Director facilitates the multidisciplinary case management process by coordinating the development and implementation of tools and systems for efficient case management.
This individual collaborates with market leaders, physicians, peers, community colleagues, system leaders, and others to identify opportunities for continuous improvement in patient care and services, executing collaborative practices that embodies commitment to patient-centered care and community engagement.
Requirements
- Master’s Degree in a health-related field
- Current Washington State RN Licensure
- Eight (8) years of clinical experience, with a preference for acute care case management experience
- Five (5) years of healthcare leadership experience
- Experience in risk mitigation, financial management, and reimbursement
