Job Description : Connect Care is Intermountain Health's direct-to-consumer telehealth service that provides 24-hours-per-day, 7-days-per-week, 365-days-per virtual urgent care through video to patients throughout Utah, Idaho, Nevada, and Montana.

We are seeking a nurse practitioner for a full-time nightshift role.Providers will diagnose and treat patients with low acuity urgent care conditions (bronchitis, UTI, rhinitis, etc.

through telehealth technologies. Providers will also provide appropriate triage and care coordination for patients with more serious medical problems or who require an alternate level of care.

In all interactions with patients and providers, team members will provide excellent patient care and ensure a superior customer service experience.

The position is accountable for the provision of patient care and other activities as permitted by licensure and defined within Credentialing and Privileging documentation (hospital), Credentialing and Scope of Practice (Medical Group), Consultation and Referral Plan (DOPL) or other similar documentation as appropriate.

As part of that patient care, this position is accountable to : deliver optimal patient care through effective clinical practice;

education of the patient, patient's family, clinical staff, and others; consultation with other healthcare providers; and maintain current knowledge regarding options for optimal patient care and outcomes.

ScopePractice within scope of licensure (defined for APRNs in the Utah Nurse Practice Act / Rules (58-31b, R156-31b)), national certification, education, training, and consistent with Intermountain Healthcare approved privileges and / or other aspects of the credentialing process.

Job EssentialsClinical Practice : Perform patient assessment to obtain a complete and accurate record of the patient's clinical information and medical symptoms.

Order tests to obtain additional information needed to ensure proper and complete care of patient.Based on assessment, determine a plan of care according to Intermountain guidelines.

Appropriately triage, treat and coordinate the care of patients presenting for Telehealth evaluation consistent with appropriate prescriptive practices, Intermountain Healthcare guidelines, and generally accepted standards of care.

Triage patients, when necessary, to a more appropriate location / level of care.Appropriate and timely documentation of clinical care.

Code and bill accurately per established protocol and procedure. Ensure that records are detailed and complete, and comply with related standards and regulations.

Perform diagnostic and / or therapeutic procedural care as defined by Scope of Practice.Assess patient to identify opportunities to coordinate, collaborate and integrate care within the broader Intermountain network and community.

Understand performance, proper use, and documentation of CLIA-waived point of care testing and Telehealth equipment.Represent Intermountain as a high quality, patient-oriented healthcare system.

Provide cost-effective treatments aligned with Intermountain standards as applicable.Education : Conduct, coordinate, and promote education to increase the knowledge of patient, patient family, and others.

Provide appropriate instruction based on patient needs to support the continuum of care and medical home.Stay abreast of current trends in telehealth and healthcare practices, and participate in continuing medical education to ensure optimal clinical knowledge and effective patient care skill.

Assist in education / promotion of Connect Care / telehealth technology, clinical application, and proper use to patients and Intermountain clinicians and staff.

Consultation / Collaboration : Coordinate and collaborate patient care efforts with other care providers.Communicate clinical information promptly, accurately, thoroughly, and appropriately to other providers of patient care.

Collaborate with other care providers in the effective sharing of practice expertise.Operate under appropriate clinical supervision based on advanced degree, regulatory requirements, and Intermountain Healthcare practice patterns.

Utilize on-call physician resources appropriately.Research : Participate in research and utilize evidence-based practice to promote quality patient care, protocol development, care process model development, and best practice standards.

Support professional development through research processes.Assist in the planning, development, implementation and coordination of Telehealth activities (potentially including making recommendations for program changes, development / implementation / coordination of new clinical areas, development of outcome and quality measures.

Minimum QualificationsAPRN degree from an accredited institution.Current licensure as Advance Practice Registered Nurse (APRN) within state of practice;

eligibility for licensure in Idaho, Utah and Nevada.and -Eligible for State Controlled Substance License II through V.and -Eligible for Federal DEA Certificate II through Vand -Basic Life Support Certification (BLS) for healthcare providers.

Clinical competencies : Adult and pediatric care, women's health, triage, patient education.and -Communication written and verbal skills (comfortable on camera, calm, empathic).

and -Flexibility, problem-solving skills, ability to work independently or as part of a team.and -Working knowledge of video-conferencing technology.

Preferred QualificationsAdditional relevant clinical experience or competencies.and -Bilingual (Spanish preferred).and -3 years of APRN clinical experience in a relevant setting.

and -Prior experience with Telehealth or similar technology.and -Familiarity / comfort with documentation in Intermountain Healthcare EMRs (HELP2 / iCentra).

Physical Requirements : Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

and -Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

and -Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

and -Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment.

Typically includes items of varying weights, up to and including heavy items.and -Need to walk and assist with transporting / ambulating patients and obtaining and distributing supplies and equipment.

This includes pushing / pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).
