Description

The Patient Navigator will assist patients and their families in managing through the vast array of services and programs offered by the hospital and the community to meet their health and wellness needs.

The Patient Navigator will provide personalized, high touch care, advocating the integration of whole person health of body, mind, and spirit.

The Patient Navigator will tailor services to the unique wellness needs of the patients served. The Patient Navigator will participate in health screenings, provide patient education, and offer appropriate referrals.

The Patient Navigator will be responsible for cultivating relationships with patients and their families, physician partners, members of the healthcare team, and community partners.

The Patient Navigator will be responsible for collection of data related to contacts and referrals as outlined by department manager.

Required Qualifications :

  • Bachelor's Degree in Social Work
  • National Provider BLS - American Heart Association upon hire

Preferred Qualifications :

1 year of experience in health care setting

Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
