Position : Physician for In-Home Geriatric Care

Location : Northern Chicagoland Area

Schedule : Monday Friday, 8 AM 5 PM

On-Call Schedule : Once every 20 weeks (telephone only, 5 PM 11 PM)

We are seeking a compassionate and dedicated physician to provide primary care to our geriatric patient population in the comfort of their homes across the Chicagoland area.

This position offers a rewarding opportunity to enhance the quality of life for seniors while working in a collaborative and supportive environment.

Key Responsibilities :

• In-Home and Virtual Care : Deliver primary care services to geriatric patients, addressing a range of acute and chronic conditions, including CHF, COPD, CKD, DM, and more.
• Patient Management : Oversee a panel of approximately 300 patients, seeing an average of 3-5 patients per day.
• Collaborative Approach : Work within a multi-disciplinary team to provide comprehensive and coordinated care.

Qualifications :

• Education : Medical degree (MD or DO) with a focus on geriatrics or primary care.
• Licensure : Active medical license in the state of Illinois.
• Skills : Strong communication skills and a commitment to patient-centered care.

Compensation and Benefits :

• Base Salary : $250,000 per year, plus mileage reimbursement at the federal rate.
• Incentives : Outcome-based quality bonus.
• Sign-On Bonus : $30,000.
