He / she functions with peers and other care providers and physicians for problem-solving, facilitating and organizing the day-to-day operations of the Case Management Department.
The person in this position coordinates, oversees, records and transmits information necessary to support the resource management of patients.
Required Minimum Knowledge, Skills, and Abilities (KSAs) Education : Associates degree preferred. License / Certifications : N / A Experience : Minimum of two years' experience in a medical setting preferred.
Basic medical terminology required. Strong customer service skills and good verbal and written communication skills. Able to work productively in a team structure and capable of working with multiple tasks simultaneously Computer skills : Microsoft Office products proficiency.
Some knowledge of community agencies / resources.
