We have an opening at our Sand Springs office . The role of a Care Coordinator II requires a Bachelor's degree in a behavioral health related field.
Minimum of one year work experience with children in the mental health field or related social service field. Must be able to provide varying types of case management and / or rehabilitative services in various setting to individual, families and groups as well as provide crisis intervention services, as assigned.
The starting salary for a Care Coordinator II is $49,201.67

Special Knowledge, Skills and Abilities :
- Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organizes and presents facts effectively.
- Ability to establish working relationships with clinical, administrative, and support staff in order to develop professionalism and coordination within the team.
- Must possess good working knowledge of community social service resources, basic crisis management skills, behavioral health resources, ability to provide trauma-informed care and a commitment to the Sanctuary Model components.
- Ability to communicate clearly and in an emotionally intelligent manner with clear, polite, and respectful communication with staff, youth served, administration, public school personnel, and the community- in person, through written correspondence and on the telephone.
- Must demonstrate professional and ethical behavior as well as the ability to maintain strictest confidentiality guidelines for interactions regarding client information at all times.
- Such specialized training and / or certification in mental health services or related field as may be required in order to provide adjunctive services, i.
e. Child / Adult Case Management Certification; BHRS, if requested.
Ability to work well under pressure meeting deadlines with efficiency and consistency while maintaining a balance between good physical and mental health and the job requirements.
Example of Position Responsibilities :
Coordinates behavioral health referrals, follows up to ensure linkages to provide targeted support services (behavior management, daily living skills, social skills);
linkage with community supports; referral to local needed services; advocacy; service coordination; and follow-up on behalf of client and their family who are experiencing emotional and / or behavioral challenges that are significantly interfering with their success at home, school, work or in the community.
- Serves as primary liaison to health part of core team.
- Coordinates input from all team members into the comprehensive treatment plan.
- Ensures that each member is aligned with a PCP.
- Provide targeted and continuous follow-up to appropriate, least restrictive and least intrusive services.
- Provide basic social skills training in such areas as : anger management, feelings identification, positive interpersonal communication, impulse / self-control, healthy relationships, and other related coping skills, using a best-practice curriculum.
- Initiating services through a non-diagnostic interview, completing a strengths / needs assessment, and developing a service plan with the client and his / her family to assess service needs.
- Coordinating services between agencies and developing cooperative working relationships with agencies serving children and families.
- Provide immediate crisis intervention on a 24-hour basis.
- Responsible for providing direct / in direct case management and / or rehab services to maintain minimum agency productivity requirement of 25 hours billable service per week.
- Support and promote a service environment for individuals in need of behavioral health and / or co-occurring services that is trauma-informed, recovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives.
- Decrease of inpatient stay through alternative service planning and coordination of follow-up referrals.
- Responsible for teaching about and implementing regular community meetings and safety plans with client(s) as part of commitment to the Sanctuary Model of client care.
Other Duties : Perform other duties as assigned by Supervisor.
Physical Requirements : Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures / programs, enter data to various programs.
Must be able to climb stairs and access non-handicapped equipped residences / facilities.
