The remote Clinical Case Manager will make outbound calls to active patients to administer assessments in order to provide more comprehensive care.
Pay : $24 / hr

Location : REMOTE CENTRAL TIME ZONE

Responsibilities :
- Make outbound calls and administer assessments to patients via phone.
- Hit daily / monthly benchmarks to achieve goals.
- Call 50+ patients per day.
- Document daily patient encounters and contact attempt notes (per patient).
- Maintain ongoing, open lines of remote communication with your supervisor and other staff
Requirements :
- Minimum 3-5 years of experience working in a high-volume call environment.
- Must have CNA, MA, LPN or similar certification
- Must have working experience within the healthcare or medical industry and a strong understanding of HIPAA compliance standards.
- Spanish bilingual is strongly preferred
- Remote and / or Work from home experience required.
- Must have a quiet, distraction free area to work from.
- Must have reliable high-speed internet.
- Must show high level of personal integrity and ability to maintain confidential information
- Excellent oral and written communication skills
- Proficient in Microsoft based software and various web-based software and technology
This is a fully REMOTE position; all applicants must be available Monday Friday 8 : 00 AM-5 : 00 PM. Please note there are no evening, overnight or weekend hours available.
