We are seeking a dedicated and skilled Hospice Clinical Liaison, LVN to join our team at Parentis Hospice.
The Clinical Liaison will serve as a vital link between patients, healthcare providers, and hospital administration, ensuring seamless coordination of care and optimizing patient outcomes.
This role involves building and maintaining relationships with referring physicians, providing clinical expertise, and facilitating effective communication between all stakeholders.
Key Responsibilities :
Patient Care Coordination : Facilitate patient admissions, transfers, and discharges by coordinating with physicians, nurses, and other healthcare professionals.
Ensure that patients receive timely and appropriate care.
Relationship Management : Develop and maintain strong relationships with referring physicians, specialists, and community healthcare providers.
Act as a point of contact for inquiries and concerns related to patient care.
Clinical Expertise : Provide clinical guidance and support to healthcare providers and staff.
Share knowledge on best practices, treatment protocols, and hospital services to enhance patient care.
Communication : Serve as the primary liaison for communication between patients, families, and the hospital.
Address and resolve any issues or concerns related to patient care and services.
- Education and Training : Conduct training sessions and educational workshops for hospital staff and community partners on hospice policies, procedures, and services.
- Quality Improvement : Collaborate with the quality assurance team to identify areas for improvement in patient care and hospital processes.
Participate in initiatives to enhance patient satisfaction and clinical outcomes.
Documentation and Reporting : Maintain accurate records of interactions, patient care processes, and referral patterns.
Generate reports and provide feedback to hospital administration on trends and areas for development.
Compliance : Ensure that all patient care activities adhere to regulatory standards and best practices.
Qualifications :
- Education : Associates degree in vocational Nursing (LVN).
- Experience : Minimum of 3-5 years of clinical experience in a hospital, hospice or healthcare setting.
Previous experience in a liaison or coordination role is highly desirable.

- Strong clinical knowledge and understanding of hospital procedures and services.
- Excellent communication and interpersonal skills with the ability to build relationships with diverse stakeholders.
- Proficiency in using electronic health records (EHR) and other healthcare-related software.
- Strong organizational and problem-solving skills, with the ability to manage multiple tasks and prioritize effectively.
- Ability to work independently and as part of a team in a fast-paced environment.
- Licensure : Valid and active state LVN license.
