Key Responsibilities :

  • Assessment : Evaluate clients' needs and strengths.
  • Case Planning : Develop personalized care plans.
  • Advocacy : Assist clients in accessing services and asserting their rights.
  • Resource Coordination : Connect clients with relevant community resources.
  • Crisis Intervention : Provide immediate support during emergencies.
  • Monitoring and Follow-up : Track progress and adjust plans as needed.
  • Collaboration : Work with other professionals to coordinate care.
  • Documentation : Maintain accurate records.
  • Education and Empowerment : Provide support and build skills.
  • Ethical Practice : Adhere to professional standards.
  • Cultural Competence : Respect diversity and ensure culturally competent services.

Qualifications

Qualifications : To excel in this role, you should have :

  • Clinical Expertise : A bachelor's degree in counseling, psychology, or a related field.
  • Therapeutic Skills : Convey empathy and compassion to those experiencing pain, physical or emotional distress or grief.
  • Communication : Strong communication and interpersonal skills.
