Who We Serve

Pacific Clinics serves children, transitional-age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education, and early childhood education programs to Medi-Cal-eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.

JOB SUMMARY

Works within the values and clinical philosophy of the agency. Under the general supervision supports Program Managers-Managed Care and Program Supervisors-Managed Care by ensuring quality comprehensive care management assessments (including risk, social determinants of health) and plans in managed care programs consistent with the requirements of CalAIM.

RESPONSIBILITIES AND DUTIES

  • Apply advanced skills to gather assessment information in development of the care plan and applies information and resources to ensure quality care.
  • Understands and applies multi-modal approaches and perspectives to whole-person care.
  • Actively participates in the identification of team opportunities for improvement or identification of deficiencies and in the development of strategies to address or bridge gaps.
  • Participates in Quality Improvement (CQI) activities and project implementation.
  • Performs other related responsibilities, as assigned, to support specific department / business needs.

Minimum Education and / or Experience

  • Master's degree in social work or a closely related field (, Marriage, Child and Family Counseling or Psychology) required.
  • Three () years of supervised clinical experience.
  • Licensed according to California state law (Welfare and Institutions Code section

PREFERRED QUALIFICATIONS

  • Minimum of years of clinical experience with complex care
  • Experience with care management plans.
  • Knowledge of Social Determinants of Health and Crisis management experience preferred.

HIGHLY DESIRABLE SKILLS

  • Experience, knowledge, skills, and abilities with culturally diverse populations.
  • Speak, read, and / or write another language.
