Licensed Transitions of Care Manager

A company is looking for a Transitions of Care (TOC) Care Manager.

Key Responsibilities

Engage with patients during hospitalization to ensure safe transitions of care and update care management plans

Collaborate with hospital care teams and community organizations to address patient needs and barriers to care

Document patient activities and outcomes, and assist in leading transitions of care training for staff

Required Qualifications

Bachelor of Science in Nursing or Master's Degree in Social Work (LSW / LCSW) with current state licensure

3-5 years of experience in care management or related fields, including safety net / public health settings

Knowledge of population health systems and disease management

Experience with electronic information systems (EHRs, care management platforms)

Bilingual in Spanish preferred
