Job Description

Primary Counselor position at our Mid Valley Outpatient treatment program (www.socialmodel.com). The Primary Counselor provides treatment services through individual counseling, group process, multi-family sessions, educational groups, and recreational activities.

• Role model appropriate healthy and ethical behaviors in all interactions, including HIPAA and Confidentiality standards.
• Maintain a primary caseload of participants, assisting and advocating for them in meeting their unique needs within the program structure.
• Develop a treatment plan for each participant on the caseload and evaluate their progress in meeting the goals and objectives within the treatment plan.
• Update the individual treatment plans, as appropriate, in a timely manner.
• Document the services provided according to standards set by the Agency and applicable County, State, and Federal regulations.

Documentation should be clear, concise, and completed in a timely manner.

• If determined by the Program Director, maintain and update program curricula.
• Facilitate full utilization of service units, including Case Management, as directed by the supervisor.
• Assist with participant’s benefits (e.g. DPSS, SSI, Medi-Cal) application(s).
• Familiarity and adherence to all regulatory expectations and ethical standards and Agency policies and procedures, including the Agency’s Core Beliefs.
• Maintain constructive relationships with family members, community members and organizations, human services agencies, and other professionals.
• Assist in ensuring a safe and alcohol / drug-free environment.
• Perform all other duties as assigned.

Credential required (salary commensurate with experience and credential) : SUD registered or certified

Non-smoker preferred
