Work Location : Miami-Dade County

Salary Range : $40,000 to $45,000 (non-negotiable)

Position Summary

The UPLIFT Service Partnership is a network of service providers working together to provide coordinated services to families in communities where children are at risk of child abuse or neglect;

poor school attendance; poor academic performance; adverse behaviors and exposure to violence. The overall goal is to reduce child risk factors, increase child and family protective factors, increase parental and family functioning, and strengthen lasting family supports.

UPLIFT provides meaningful care coordination, comprehensive case management, and wrap-around services to youth and families.

The UPLIFT team consists of Case Managers who, among other responsibilities, engage in community awareness and recruitment activities, triage families in need, identify needs through assessments, develop service plans, connect families to the right services, and support the family through case management until family risk factors have decreased and strengths have increased sufficiently.

Essential Duties and Responsibilities

  • Develops and maintains expertise about GA programs and services as well as community resources available to families.
  • Provides and / or coordinates critical services for children and youth, including family stabilization, service coordination, system navigation, and facilitating access to both non-traditional and agency-based supports and services.
  • Ensures all clients are properly screened and assessed to evaluate appropriateness for program participation, maintaining written documentation detailing each client contact made.
  • Assesses client needs and family strengths and formulates an appropriate and responsive plan of action that addresses identified areas of need.
  • Conducts level-of-care determinations that conform with agency and governance mandates while ensuring that clients receive the information required to make informed choices regarding program participation.
  • Ensures the maintenance of accurate, complete, timely, and high-quality client records that comply with internal and external regulatory standards and agency policies and procedures.
  • Maintains accurate and confidential client records and contact information for all families involved.
  • Conducts case coordination in a strengths-based, family-centered, family-driven, and culturally competent manner through a wraparound team-based process in which youth and families participate as partners.
  • Guides and assists participants to produce an individualized course of action (ICA) plan that reflects the families’ strengths and incorporates GA or community resources.
  • Provides orientation education to clients regarding program rules and regulations, client rights and grievance procedures, staff roles and program structure, and information pertaining to available program services and optional methods of attaining recovery goals.
  • Ensures open communication with partners who refer families and with programs that receive our referrals on progress or issues regarding the family.
  • Stays abreast of local and regional community issues, emerging trends, and best practices applicable to the work of the community service partnership.
  • Attends meetings with affinity groups representing the organization in a professional manner, as needed by GA.
  • Keeps supervisor informed of day-to-day activities, significant events or conditions impacting clients and / or staff well-being, team safety, and office operations.
  • Abides by all HIPAA rules and regulations. Maintains strict confidentiality when handling client information.
  • Serves as an Ambassador of Gang Alternative, Inc.
  • Other duties as assigned.

Qualifications and Competencies

  • Minimum of a bachelor’s degree in public administration, social work, and / or community planning or related field; Master’s Degree preferred.
  • Bilingual (Creole strongly preferred).
  • Requires excellent leadership skills in community involvement and organizations.
  • Requires an unrelenting customer service approach.
  • Excellent communication skills and ability to reach a variety of audiences.
  • Excellent organizational skills, dependent and responsive with attention to details and ability to prioritize multiple tasks.
  • Requires excellent interpersonal skills, knowledge of the community and social services in the area, and resources for substance abuse, cultural competence, and ability to work effectively with youth and adult volunteers.
  • Proficiency in Microsoft Office Suite especially Word, PowerPoint, and Excel.
  • Ability to work effectively in a team environment, with excellent interpersonal skills, a positive attitude, and maximum discretion when handling sensitive and confidential information.
  • Willingness to work flexible schedules as deemed necessary including weekends and evenings.

Note :

  • This position may require independent travel between workstations and other sites. Consequently, the incumbent must always maintain a valid Driver’s License and insurance coverage on a personal vehicle.
  • Performing the duties of this position requires the ability to walk, sit, stand, reach, talk, hear, and lift presentation materials, including equipment.
