The Housing Case Manager position exists to provide case management services to participants in the Rapid Re-Housing (RRH) programs.
The goal of the program is for participants to be able to pay full rent in a market rate unit after being in the program by receiving case management, education and employment assistance, health and wellness support, and a focus on increasing income.
Ensures participants meet the eligibility criteria of the Rapid Re-Housing program, i.e., homelessness status, income verification, etc.
and any additional needs are met while receiving YWCA assistance. These duties are performed both in a general office environment and in the field and community where program participants reside.
Position Duties & Functions
- Facilitation of Direct Services
- Facilitates direct services and housing assistance for the Rapid Re-Housing program (e.g. intake, counseling, advocacy, deposit and rental assistance, referrals, and case management, etc.)
- Facilitates screenings, assessments, safety and financial planning and referrals for all participants in the program
- Assist with participant identifying and locating housing and services that will help them implement their goals.
- Work with participants to enroll in public benefits to which they are entitled
- Teach practical financial skills, including developing a household budget, paying bills in a timely manner, and opening and maintaining a bank account
- Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies, debtors and creditors).
- Assist participants in development of a strength-based / solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency;
develop an effective, timely referral network in order to ensure ongoing direction and support as needed.
- Identify participant strengths and barriers to stability and assist participants to reducing barriers and linking to resources and services.
- Monitors data security and ensures consistent implementation daily
- Facilitates existing partnerships, collaborations, and relationships with other agencies or providers to ensure the best delivery of services to participants in our Rapid Re-Housing program (e.
g. Landlords, CILO for disabilities, Lewis Center for housing, etc.)
- Builds new partnerships, collaborations, and relationships with other agencies or providers to acquire the best services for participants in the Rapid Re-Housing.
- Facilitates at least 2 community education or professional trainings in the community annually to promote our Rapid Re-Housing Program.
- Maintains accurate, complete, and up-to date participant files and case notes; Osnium and Homeless Management Information System (HMIS) entry and all other requirements to ensure accurate statistical data.
- Prepare reports and other paperwork per established program standards.
- Work closely with staff and case managers in other programs (e.g. Harmony House, Outreach, etc.)
Skills & Abilities
- Must be knowledgeable of Microsoft Office (Word, Excel, PowerPoint, Outlook)
- Must have excellent verbal and written communication skills
- Must be prompt; timely and responsive to needs of fast-paced organization
- Must be able to handle confidential information discretely
- Must be able learn quickly and have strong problem solving and critical thinking skills.
- Must be able to work independently and with diverse populations and able to meet clients in their homes
- Requires knowledge and belief in Housing First philosophy and strategies.
- Must maintain a positive work environment
- DCF Domestic Violence Program Core Competency and privilege status obtained within first 90 days.
Education
Bachelor’s degree in Social Work, Criminal Justice, Sociology, Psychology, or related Human Service field
Experience
- Minimum 1 -year experience of case management, preferably with individuals or families impacted by homelessness, those experiencing domestic violence, or participants working impacted by trauma, providing advocacy and / or housing services and familiarity with community resources, networking and collaboration
- Minimum 1 -year experience utilizing client database systems (i.e. Osinum, Homeless Management Information System (HMIS) or similar data systems) for data entry and reporting preferred
- Familiarity with HUD Continuum of Care (CoC) Program regulations
- Bilingual preferred
- Must have a valid driver’s license
SPECIAL REQUIREMENTS AND PHYSICAL DEMANDS :
- Ability to exert physical effort which may involve some lifting, carrying, pushing and / or pulling of objects and materials of light weight (up to 20 pounds), bending, stooping, stretching, squatting, and sitting.
- Minimal travel required in the Palm Beach County area.
