Our client is seeking a Home Care Coordinator to develop and implement home care services for PACE program participants. The ideal candidate will have a passion for serving high-risk seniors and frail older adults, possess an LVN License, and have experience in managing home care services.
Key Responsibilities
Coordinate Home Care Services for participants using home care agencies
Participate as a member of the Interdisciplinary Team (IDT)
Conduct nursing care within LVN Scope of practice
Provide general nursing care in the clinic and participant’s home
Document in the EMR within set documentation timeline
Communicate with home care employees to understand participant’s condition
Collaborate with the Transportation Coordinator for participant transportation
Schedule Home Care appointments and support scheduler for other appointment needs
Oversee visits completed by external agencies
Schedule and Shift Details
Monday to Friday, 8a-5p
Travel
Occasional local travel

qualifications

- Associate degree or higher
- 3+& years of experience in& ANY& of the following :
- Home Health Care
- Home Health
- 2+& years of experience in& ANY& of the following :
- Geriatric Care
- Senior Care
- Geriatric / LTC
- Valid Licensed Vocational Nurse (LVN) in the state of California (CA)
- ANY& of the following valid licenses / certifications :
- American Heart Association Basic Life Support (AHA BLS)
- American Red Cross Basic Life Support (ARC BLS)
- Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law (yes)
