Position Details :
Hartford HealthCare is seeking a Physician on a per diem basis to provide Skilled Nursing Facility (SNF) admissions and rounding coverage.
Role : Per Diem Physician, Geriatric patient population
Location(s) : Stratford, CT
Setting : 100% Skilled Nursing Facility (SNF) patient admissions and rounding
Schedule : Per Diem, flexible scheduling
Call : No call

- Supported by team of APP’s that are in the facilities full time within a team of 18 Geriatric providers
- Strong support staff, nursing and operational team plus strong peer support to assist with any challenging situations that may arise
- Our system offers a robust compliment of senior resources and a Geriatric Council that our team works closely with to provide a personalized, coordinated care
