Responsibilities :
The Pharmacist is responsible for providing pharmaceutical services within the hospital that has a patient population ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions, with rehabilitation and / or wound care needs.
The Pharmacist duties include :
- Verification of medication orders for accuracy and appropriateness. Review of
- medication profiles.
- Assists in the resolution of drug therapy problems.
- Participation in medical and multidisciplinary rounds.
- Perceive and evaluate the medication information needs of patients and families, health care professionals, and other person.
- Use a systematic approach to address medication information needs by effectively searching, retrieving, and evaluating the literature and appropriately communicating and
