This position will work on-site in San Antonio, TX . Local candidates only.
The Position
The Social Worker is responsible for supporting Chosen's mission to help youth and families heal from trauma by using our unique, trauma-informed m odel with youth and families who have been impacted by the child well-being system.
This individual coordinates the overall plan of care for a client, from inquiry to completion, and works directly with familie s and youth while providing coaching, case management, and therapeutic resources.
The position involves a multi-disciplinary approach and includes communication with multiple professionals involved with the youth and families .
Youth and families served include high acuity youth in the child well-being system .

Job Responsibilities
- Process client applications
- Enter and maintain client contact and demographic data in client management system
- Complete clinical assessments as needed
- Create actionable plan for the client family system
- Conduct trauma-informed therapy through telehealth or in person, as applicable
- Coordinate with interdisciplinary team to meet needs of the family
- Meet with collaborating agencies to share workshop / training possibilities
- Conduct trauma-informed training to child well-being professionals as requested
- Participate in creation of trauma-informed content and resources
- Attends supervision with Program Director
- Completes documentation in a timely manner
- Provides crisis support / management as needed
- Other tasks as assigned

Education and Experience
- Master's degree in social work with 2-4 years related experience and / or training, or equivalent combination of education and experience required
- Trust-Based Relational Intervention® Practitioner strongly preferred
- Minimum LMSW or LPC-A strongly preferred
- Solid working knowledge of current industry norms and challenges
- Trauma-Informed training or certification
- Solid working knowledge of current industry norms and challenges
- Technically competent with Microsoft Office suite
- Spanish b ilingual skills preferred
- Quiet workspace and high-speed internet and phone connection to support telehealth services and video conferencing
