Intake Coordinator
Caring Hearts Homecare is looking to hire an intake coordinator, someone who is self-driven, detail-oriented, responsible, professional, and a team player.
Skills and Qualifications :
- Is computer literate in MS Word and Excel
- Is dependable, reliable, and organized
- Must be organized and able to multitask efficiently
Duties / Responsibilities :
- Manage Admissions and Referral Intake process
- Answer and respond to phones and email correspondence
- Receive all intake information and enter the appropriate information on the intake reports.
- Audit and maintain accurate intake / referral documents and reports
- Insurance verification
- Patient data entry
- Track and obtain referral / admission paperwork
- On-Call Rotation
- Performs other duties as assigned
