Schedule : Monday through Friday, 8 : 00 am to 5 : 00 pm (or similar schedule)
Location : Topeka, KS ( Kansas City, Wichita or Topeka, KS - areas with ability to travel between health center locations. )

Role Overview :
The ACO Outreach and Engagement Coordinator plays a vital role in connecting patients with the Kansas Care Network Accountable Care Organization (ACO) Health Centers.
This position is responsible for performing telephonic, written, and electronic outreach to patients, ensuring their records are accurately maintained by acquiring necessary documentation from external organizations.
In this role, you will guide and support attributed members as they navigate the healthcare system, primarily through outbound and inbound communication.
Your ability to listen attentively, respond thoughtfully, and foster a compassionate environment will be key in empowering our members to access the care they need.
Major Duties :
- Assist with scheduling appointments to address care gaps.
- Support inpatient discharge and emergency department follow-up visits for attributed patients.
- Conduct outreach to attributed patients regarding missed and / or cancelled appointments.
- Engage with attributed patients to address care gaps, ensuring completion of necessary health care screenings or evidence-based care.
- Reach out to new, medically unmanaged ACO attributed patients to establish long-term primary care relationships with Kansas Care Network Health Centers.
- Maintain accurate documentation of all outreach encounters and attempts.
- Utilize data from payers and other external organizations to enhance patient-centered outreach efforts.
- Identify trends in outreach success to help coordinate effective patient-centered outreach strategies.
- Provide health education, preventive health information, and outreach to attributed patients.
- Collaborate with Kansas Care Network Health Center teams to address care coordination needs.
- Support the Kansas Care Network team with daily tasks and operations.
- Pilot new processes and workflows to improve patient care and outreach.
- Interact with Kansas Care Network ACO attributed patients in a culturally responsive and appropriate manner.
- Protect the privacy and security of attributed patients' information.
Qualifications :
- Minimum : High School diploma or equivalent
- Minimum One (1) year of experience in community outreach, health education, or population health
- Excellent telephone etiquette. Computer use includes word processing, excel, and Electronic Health Records. Able to operate office equipment including computer, fax machine, copy machine, etc.
Able to follow through with assignments responsibly, accurately and efficiently.
- Knowledge of HIPAA, healthcare terminology, and healthcare related policy.
- Knowledge of value-based care promising practices.
- Ability to promote a professional and positive appearance and work environment.
- Strong interpersonal, organizational, written, and verbal communication skills required.
- Creative thinking and problem-solving skills; willingness to seek input regularly for process improvement.

- Ability to travel as needed within the state and to attend national meetings (limited travel is expected).
Preferred Qualifications :
- Associate Degree in Healthcare or related field
- Three (3) years of experience in community outreach, health education, or population health
- Previous experience in a health care setting
- Experience with eClinicalWorks electronic health record
- Leadership skills, understanding of cultural influences on health status and underserved populations. and a willingness to assist health professionals in improving quality care.
Physical Demands :
- Regularly maintain stationary position with regular need to relocate to attend meetings internally and externally.
- Ability to read computer screens and operate computer peripherals for an extended period or the ability to discern electronic information through available accommodations.
- Regularly communicate with staff, members, and stakeholders verbally and in writing through in-person, virtual, or other communication vehicles.
Work Environment :
Professional, fast-paced, and deadline-oriented environment in an office setting with regulated temperatures and moderate noise level.
