Intercept Health is seeking an energetic, experienced, and passionate Senior Life Skills Coach who will :
Interviewing children and families; Conducting youth assessments; Preparing and carrying out social plans with children and families;
Preparing and maintaining case records.
- Supervising youth in Independent living arrangement; Conduct weekly Community Life Skills Groups, review youth's progress with Life Skills Development, and discuss barriers with the Program Supervisor to determine appropriate activities and events.
- Coordinating services to minimize fragmentation of care, reduce barriers, and link children with appropriate services to ensure comprehensive, continuous access to needed medical, social, education, and other services appropriate to the needs of the resident;
- Preparatory counseling with youth and families for placement or discharge, or both;
- Assists Program Supervisor with monthly mandatory staff meetings as well as all scheduled in-service and other required training.
Develops monthly schedule for Evening support staff.
Education and Experience Required : The selected candidate will have
- A Bachelor's Degree in Social Work or a field related to Social Work and one year experience providing casework services to children and families OR
- A Bachelor's Degree in any field plus two years experience in providing casework services to children and families.
Preference for these positions will also be given to applicants who have experience in case management experience working with youth with emotional and / or behavioral difficulties.
CERTIFICATES, LICENSES, REGISTRATIONS :
Current Virginia driver's license, safe automobile, and current and sufficient auto insurance. Your driver's license report must comply with the following : A record of -5 or better driving points, no more than 3 accidents or moving violations within the last 3 year period, no driving under the influence (DUI), reckless driving, negligent homicide, hit and run, driving on a suspended license, or permitting an unlicensed person to drive violations within the last 3 year period.
