Responsible for the delivery of servicesby staff assigned. # EDUCATION / EXPERIENCE : Registered Nurse with anactive license to practice nursing in Connecticut and either : Amaster degree from a program approved by the National League forNursing or the American Public Health Association with a minimum ofone year clinical experience in a Home Health Agency or Abaccalaureate degree in nursing and a minimum of 3 years of fulltime clinical experience in nursing, at least one of which was in ahome health agency or A diploma in nursing or an associates degreein nursing with A.

a minimum of 3 years of full time clinicalexperience in nursing within the past five years, at least one yearof which was in a home health agency and B.

Evidence ofcertification by the American Nurses# Association as a communityhealth nurse or completion of at least six credits received withintwo years in community health nursing or six credits in health caremanagement from an accredited college or university program orschool of nursing.

Certificate / License : Possession of a currentlicense to practice as a registered nurse in the State ofConnecticut.# Possession of an individual malpractice insurancepolicy.

Responsible for management of professional andparaprofessional nursing services as well as those provided bycontract to hospice patient families.

Responsible for management ofprofessional and paraprofessional nursing services by assessmentand evaluation of community needs, agency needs, and agencyresources.

Makes recommendation to the Hospice Director on staffingneeds and requirements. Assures clinical competency of staffassigned.

Responsible for accurate and timely assessment andevaluation of performance of staff assigned. Assures staff maintainqualitative standards and comply with Agency quality assurancegoals.

Encourages the professional and clinical development ofstaff assigned. Assures staff are knowledgeable in criteria for allpayer sources, as appropriate.

Participates in weeklyInterdisciplinary Team Meetings and updates staff on new trends andissues. Provides ongoing case evaluation to assure that clinicaloutcomes, documentation, and billing requirements are met.

Reviewsprogress on clinical outcomes to assure efficient and effectiveachievement of goals. Provides ongoing audit of patients# recordsto assure compliance with documentation and regulatoryrequirements.

Contacts insurance companies, as appropriate, withclinical updates. Assists with problem solving related to billingproblems.

Conferences with Hospice RN and other disciplines todiscuss patient care, progress on goals, coordination of services,and plans for ongoing service or discharge.

Assures timelycompletion of documentation requirements. Maintains qualityleadership and management goals and objective. Facilitates opencommunication between staff and senior management.

Activelycommunicates and works collaboratively with internal and externalcustomers. Implements agency policies / procedures goals andobjectives with Hospice staff.

Actively communicates and workscollaboratively with internal and external customers, including butnot limited to physicians, town representatives, hospitals, networkpersonnel, nursing homes, Hospice Program Director.

Is accountablefor patient utilization and staff productivity, and conferenceswith appropriate staff to address variances.

Accepts and processpatient referrals as needed. Participates in the hiring,interviewing, orienting and terminating of staff for areasassigned.

Participates / facilitates agency committees, focus groups.Manages other agency programs, as assigned.
