As a Housing & Service Coordinator at the Hampton-Newport News Community Services Board, you will play a crucial role in ensuring continuity of care for individuals living with mental illness, as well as those living with co-occurring intellectual developmental disabilities and substance abuse issues.
Your responsibilities will include helping clients identify their housing needs and assisting in their applications for housing opportunities, all while developing comprehensive housing plans tailored to their unique situations.
In this role, you will manage a diverse caseload, addressing clients' physical, financial, social, and personal needs. Your service coordination expertise will encompass outreach, psychological assessments, treatment planning, advocacy, monitoring, and counseling services.
Additionally, you'll provide essential crisis intervention, daily living support, and medication supervision, ensuring that each client receives holistic care.
Transportation for clients may also be part of your duties, enhancing their access to necessary resources and support.

To excel as a Housing & Service Coordinator at the Hampton-Newport News Community Services Board, candidates must possess a Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.
A minimum of one year of experience in case management or service coordination, particularly involving housing resources for adults with mental health issues, developmental disabilities, and substance use disorders, is essential.
Successful candidates must be a Registered Qualified Mental Health Professional - Adult (QMHP-A) with the Virginia Board of Counseling or register as a QMHP-Trainee within the first 30 days of employment.
Proficiency in utilizing various software tools for case management and documentation will also be necessary.
Additionally, a valid Virginia Driver's License is required to facilitate client transportation, reflecting the commitment to providing comprehensive support for those in need.
Strong communication, empathy, and organizational skills are vital to effectively managing client needs and developing personalized care plans.
The knowledge and skills required for the position are :
- Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling
- One (1) year of experience in case management or service coordination services, including housing resources for adults living with mental health developmental disabilities, substance use, and co-occurring disorders.
