Job purpose :
Serve as Transition Manager to assigned caseload / worksite. This may include but is not limited to career counseling, crisis intervention, individual counseling, advocacy, modeling behaviors, meeting site specifications, and safety control procedures.

Duties and Responsibilities :
- Develop curriculums and facilitate community employment activities to prepare for employment within three years of entry to the program.
- Maintain accurate records, datasheets, IPP's, case notes, and other program recordings according to agency standards.
- Supervise Transition staff ensuring all rehabilitation and employment activities are community-based.
- Receive supervision from the Supported Employment Manager. Much of the daily operations with individuals and staff must be coordinated without direct supervision.
- Provide immediate crisis or counseling intervention where necessary.
- Provide direct supervision of individuals in an employment / community setting to include teaching new skills, developing natural supports, reinforcing good work habits, establishing rapport with on-site employer, compliance with all work specifications, and quality control procedures.
- Teach Financial Management, Health & Fitness, Networking, and Transportation as it related to employment.
- Develop employment exploration sites, internships, and volunteer sites.
- Establish and implement employment goals for all participants.
- Develop business networks, job shadowing, and informational interviews.
- Complete accurate reports and documentation in a timely manner including attendance, data, and waiver billing.
- Maintain open communication with outside agencies as relevant to the needs of individuals served.
- Participate in in-service and agency committees.
- Oversee special projects within Transition Services.
- Meet all DDS, and BRS / DORS standards for all files managed.
- Complete audits and time studies as assigned.
- Other related duties and responsibilities as assigned.
Qualifications :
- High School Diploma.
- Bachelor's Degree in Rehabilitation Counseling or related field preferred.
- Three years of Vocational Rehabilitation experience with particular emphasis on job development preferred.
- One year of experience working with persons with disabilities is preferred.
- Valid Driver's License and ability to possess a state of CT Public Service License.
- Must maintain a satisfactory driving record.
- You are required to be in possession of a car on-site during your entire shift to transport yourself and / or individuals served to community sites in all weather conditions and throughout all neighborhoods.
- Depending upon the assignment must be able to work in a variety of settings, weather conditions, and locations.
- Must maintain PMT and CPR certifications.
- Knowledge and skills in Microsoft Office and the internet.
- Work a flexible schedule that may include early mornings, evenings, weekends, and / or holidays.
- Ability to transfer individuals requiring personal hygiene care.
- Provide physical interventions which may include lifting, mobility, first aid, and safety evacuations.
- Have physical endurance to use physical crisis intervention techniques which may include restraining individuals.
- Perform and model a variety of occupations.
- Communicate clearly in oral and written form on a constant basis.
- Do calculations and math.
Physical Requirements :
- Move and lift weights up to 40 lbs. on an occasional basis.
- Work independently; utilize good judgment and possess a strong sense of initiative.
