A little more about our center :

Southview Manor is an inner-city skilled nursing facility (SNF) with multiple levels of care, including long term care and a large behavioral health (formerly indigent) population.

• 5 Star Quality measure
• The average age of our patients is 35-55 and the most common diagnoses are schizophrenia, bi-polar and major depressive disorders.
• Many are able to age in place, and about 25% return to the community.
• We have several programs encouraging engagement, but the most popular spot in our center is the game room.

The National Association of Rehab Providers & Agencies ( NARA ) awarded Transitional Care Management their "Impact Award" for 2023 for our Life Skills Programming.

Job Description

What you will do in this role : When you are a patient, games and entertainment can be the highlight of your day. As an Activity / Activities Director you will design a curriculum to keep our short-term residents interested & engaged in their recovery and rehab.

For our long-term residents, you will create life enrichment events, often focused on cognitive and memory retention. This could involve leading exercise groups, managing a team of Aides, and above all, making this a stimulating environment for all.

Qualifications

Activity / Activities Director : Officially we title this job an Activities Director, but in other centers it may be called a Life Enrichment Coordinator.

What we need from you :

• Certified Activity Director (the 36-hour certification course for the state of IL)
• Prior experience in a healthcare setting as an Activity Director is required
• Extensive involvement with leading a dynamic team, creating an engaging events calendar and managing the individual needs of long-term care patients that address their physical, cognitive and social deficits
• Electronic medical records experience (Point Click Care)
