The Site Lead Registered Nurse (SLRN) is responsible for guiding and supervising assigned nursing personnel in delivering patient care.

The SLRN will collaborate with nurses, physicians, and other healthcare professionals in the delivery of patient care. Assumes responsibility for the management of a selected caseload of patients as defined by the clinical practice area.

Responsibilities Include But Are Not Limited To

• Performs assessments, data collection and modifies patient care in an ongoing and systemic manner, focusing on urgency including physiological, psychological, psychosocial, and cognitive status of patients based upon symptoms and department standards of care.
• Responsible for performing in-person and phone triage.
• Performs complete documentation responsibilities relating to patient's medical records in a timely, accurate, and concise manner.

Assesses and documents conditions, to include skin integrity and vital signs.

• Formulates patient-specific goal-directed plan of care which is prioritized and based on nursing assessment and patient outcomes, including seeking the participation of the patient and significant others.
• Ensures that total patient care needs are met by following an established multi-disciplinary plan of care. Implements care in a knowledgeable, skillful, consistent and holistic manner, including appropriate patient follow-up.
• Utilizes knowledge of dosage calculations, indications, actions, routes, side effects and patients’ rights, for proper administration of medications.

Obtains appropriate consents before administration of vaccines / other medications as appropriate.

• Assists in recruitment with Nurse Manager and helps with orientation, training and retention of new staff (LPN’s and MA’s) and the maintenance of their competency.
• Works with Providers to assess performance of LPN’s and MA’s.
• Performs all other duties as required of the position and requested by leadership team.

Requirements

• Graduate from an accredited school. Must have a current Registered Nurse Connecticut State Licensure. CPR certification required.
• Minimum of one (1) year experience in a long-term care facility or rehabilitation center. Clinic experience is highly desirable.
• Excellent Patient Management skills.
• Must be strongly motivated and able to accept supervision.
• Computer skills are essential with the ability to learn EMR and EPIC medical records.
• Ability to provide phone triage along with knowledge of referral process, medication refill procedures and administration.
• Knowledge of CLIA waived lab testing, Urine Dipsticks and Glucometers are necessary.
• Excellent leadership qualities and knowledge of management skills are necessary.
• Bilingual individual is desirable, but not required.
