Summary :
The Ponca Tribe of Nebraska is a federally recognized Tribe with its ancestral homeland located in Niobrara, NE. The Tribe does not have a reservation in the classic definition;
rather, the Ponca Restoration Act defined its jurisdiction as a 'Service Delivery Area' comprised of 15 counties throughout parts of Nebraska, Iowa, and South Dakota.
Within its Service Delivery Area, the Tribe offers a broad range of health, social, educational, and cultural services.
Essential to the mission of the Ponca Health Services, the Integrated Health Care Therapist will work with relevant stake holders to provide holistic and integrated health careby supporting the collaborative care team, primary care providers, patient, and behavioral health department through regular consultation on an assigned caseload of patients.
The Integrated Health Care Therapist advises the primary care provider on appropriate treatment strategies, manages referrals, provides brief interventions, educational support, and diagnosis and management of behavioral health and substance abuse conditions.
They will be devoted to providing both mental health and substance abuse counseling with an emphasis on culturally sensitive interventions to individuals and families.
Essential Responsibilities and Duties :
- Provide consultations with PCPs to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans.
- Identifies and evaluates health risk behavior(s) and provides brief, targeted interventions to support optimum mental, behavioral, emotional, and physical health, in accordance with the preferred care model at Ponca Health clinic sites.
- Works cooperatively with clinic staff to provide patients with referrals for appropriate specialty.
- Make referrals for community mental health resources as appropriate.
- Maintain a therapy caseload utilizing Evidence Based Treatments.
- Provide outpatient counseling to individuals and families needing short or intermediate term mental health interventions.
- Provide outpatient group therapy, as needed.
- Conduct mental health, substance use, and co-occurring assessments and provides recommendations
- Complete all charting as needed and required by contracted funding sources : including assessments, progress notes, and billing.
- Participate in weekly case huddles.
- Participate in weekly supervision and case consultation with Behavioral Health Team
- Other duties as assigned, including, but not limited to : Assisting in provider training and education in integrated behavioral health-primary care model, assisting with grant data collection, attending rant recipient award meetings, Community Outreach, and participating in Behavioral Health quality improvement activities.
Relationships and Authority :
This position has no supervisory authority.
Working Environment and Physical Demands :
This position is in professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, scanners, and photocopiers.
Qualifications : Minimum :
Minimum :
- Master's degree in social work or counseling.
- Provisionally Licensed Mental Health Provider
- Excellent people skills and ability to work collaboratively with primary care staff, patients, and manage behavioral health clinicians.
- Computer proficiency in Microsoft Word, Excel, and PowerPoint programs.
- Excellent written and oral communication skills.
- Valid driver's license and be insurable.
- Successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.
Preferred :
- 2+ years' experience in providing individual, family, and group counseling.
- Fully licensed or Independently Licensed Mental Health Provider
- Licensed or Provisionally Licensed Drug and Alcohol Counselor
- One year experience working with Native Americans or Native American organizations.
- Knowledge and understanding of Public Law 93-638" Indian Self-Determination Act.
