Position Summary :
The Special Education Transition Facilitator will use structured techniques to support students with disabilities in obtaining the skills needed for competitive, inclusive employment leading to more positive school and post-school outcomes. Essential Duties :
- Provides instruction using district-approved curriculum related to self determination including setting goals, making decisions, developing plans to reach goals and tracking progress towards goals.
- Use assessment data to identify students’ areas of strength and areas in need of instruction.
- Track students’ progress in the development of self-determination skills over time.
- Report student, classroom, school and district self-determination status and outcomes.
- Provide students opportunities to develop and practice using skills associated with self-determination.
- Individualize instruction based on student needs, including students with complex communication needs.
- Involve families in order to integrate a family’s unique preferences, beliefs and values.
- Collaborate with Administrators and School Staff in order to schedule students in a manner that minimizes disruption to the learning environment.
- Problem-solve obstacles to student engagement.
- Assist the Office of Exceptional Student Services Leadership Team in monitoring local plan implementation, legal compliance and data collection.
- Establish and maintain professional communication with parents, students, and district personnel; ensure that all interactions are supportive, courteous, and respectful.
- Attend and participate in department meetings and serve on staff committees as required.
- Pursue professional development activities that align with district goals and initiatives and current professional research.
- Comply with policies established by federal and state law, State Board of Education rule, and district board policy in the areas of assessment, placement, and planning for Special Education services.
- Comply with all district and local campus routines and regulations, and verbal and written directives.
- Perform other duties as assigned.
Minimum Qualifications (Knowledge, Skills and / or Abilities) :
- Associates in Arts Degree (AA) is required. Bachelor’s Degree is preferred.
- Experience in the education setting is preferred.
- Possession of a valid operator's driver's license and acceptable driving record as determined to meet district motor vehicle insurance coverage requirements.
- Relevant experience may be substituted for formal education at a 2 : 1 ratio. Therefore for every 1 year of college lacking, 2 years of directly relevant experience may be substituted.
Physical / Mental Requirements :
- Must have adequate manual dexterity to write legibly and perform required duties on the computer.
- Must have adequate visual acuity to read, interpret and transcribe written material and other required duties.
- Requires normal range of hearing and clear speaking abilities to interact appropriately with others in person and on the telephone.
- Requires some stooping, bending, stretching and occasional lifting not to exceed 25 pounds.
- Sitting for prolonged periods of time.
- May periodically require work outside of normal business hours, including weekends, under sometimes. stressful conditions in order to meet business needs and strict deadlines.
Work Environment :
- Office duties will be performed in a well-lighted, climate controlled environment.
- Use of personal vehicles for travel within and outside the district.
