Michael's House is currently seeking a dynamic Patient Care Coordinator - full time. The Patient Care Coordinator is responsible for the coordination of administrative and discharge planning of patients.

Qualifications
Essential Duties and Responsibilities
- Assist the patient in coordinating immediate outside concerns such as legal obligations, employment and / or disability documentation.
- Works with the patient and treatment team to develop a comprehensive aftercare plan.
- Documents all interactions with patients within the electronic medical record
- Provides communications / updates to the treatment team, families, and other referral resources
Education and Experience
- Bachelor's degree in psychology, social work, or related field required. Master's in a related field preferred.
- Minimum 2 years' experience in a mental health / healthcare field preferred.
- Licensed or certified as a chemical dependency counselor or registered to become licensed or certified within 6 months of start date.
