The Urology Group is searching for LPN or RN to work as a Patient Navigator in our Norwood office. This is an administrative position.
The hours are full time, Monday-Friday, 8am to 4 : 30pm.
Summary : Responsible for Overactive Bladder (OAB) patients
Essential Duties and Responsibilities include the following :
o Follow OAB pathway
o Maintain OAB Database in Clinical I
o Data mine from a weekly report for eligible patients
o Follow patient from diagnose to satisfaction of treatment
o Schedule office appointments
o Send out OAB surveys
o Order procedures on doctor’s behalf
o Fill prescriptions on doctor’s behalf
o Document all activity in patient’s chart in Integra / Database
o Point of contact for patient going forward
o Attend OAB dinners, OAB committee meetings, and any procedure meetings
o Back up other pathways as needed
o Performs other duties as assigned

Requirements :
- Valid Ohio LPN / RN license.
- Ideal candidate possess excellent critical thinking, problem solving, communication and customer service skills.
- Must be dependable, self-motivated, detailed oriented, and well organized.
- Must be able to work independently with multi-tasking skills.
- Previous experience with Allscripts and EMR a plus.
