Job Summary :
The Home Care Manager at Griswold is a full-time position within the healthcare industry. As an individual contributor, the Home Care Manager will play a crucial role in ensuring the efficient and effective delivery of high-quality home care services to clients.

Responsibilities :
- Manage a caseload of home care clients, including but not limited to scheduling, coordinating care services, and conducting reassessments as needed - Manage client inquires through emails, calls and in-person interactions with professionalism and compassion.
- Communicate with clients and their families to establish care plans and ensure their needs are being met
- Recruit, interview, hire and educate a team of home care providers to deliver exceptional care services
- Monitor and ensure compliance with all state regulations and company policies
- Coordinate with healthcare professionals and other community resources to provide holistic care for clients
- Maintain accurate and timely documentation of all client care activities
- Support Brand Awareness and attend meetings and make contact with potential referral sources, willingness to travel within the territory as needed and access to an automobile and required insurance as well as developing contracts with referral sources
- Collaborate with the Griswold team and Managing Director to identify areas of improvement and implement strategies for enhancing the overall quality of care services - Other duties as assigned
Requirements :
- Bachelor's degree in healthcare management or a related field preferred
- Minimum of 2 years of experience in home care management or a similar role
- Knowledge of state regulations and policies related to home care services
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize tasks effectively
- Proficiency in computer skills, including electronic health record systems
- Must be able to work independently and remotely with minimal supervision; be a self-starter
