40 Hours / week, Full-time, Day shift
Maine state licensure to practice as a Licensed Social Worker (LSW), Licensed Social Worker - Conditional (LSW Conditional), MHRT-C, or Provisional MHRT / C is required.
The Discharge Planner role hasthe responsibility to carry out a full range of aftercare and discharge planning services. The specific discharge planning needs of each patient shall be identified in collaboration with the unit treatment team and may include patient interviews, record reviews, and participation in network and discharge planning meetings.

Applicants well versed in the eligibility criteria for HCT, CRCF and section 28 services with Acentra are preferred.
The role of the Discharge Planner is to maintain comprehensive knowledge based of agencies, organizations, and resources within the referral community.
The Adult Discharge Planner has the responsibility of assisting the assigned social worker with securing housing, assisting in completion of Maine Care applications, case management or other community supports for those patients who require special assistance or have significant discharge planning needs, which may extend their hospitalization.
The Children / Adolescent Discharge Planner will assist the assigned social worker in completing of required residential placement applications (ITRT) and case management or other community supports for those patients who require special assistance or have significant discharge planning needs, which may extend their hospitalization.
The Discharge planner shall also provide reciprocal relations with county providers by serving as a liaison from the hospital and by making regular visits to various community programs when applicable.
They shall also be expected to provide support to units when the social worker is unable to be there.
The Discharge Planner will support the hospital's Performance Improvement Plan by actively participating in multidisciplinary performance improvement teams.
This individual will contribute to the ongoing continuing education programs and effectively communicate with all customers, both internal and external.
Actively participate in Gemba presentations, development of team key performance indicators, data collection and problem-solving to improve quality and outcomes.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education : Bachelor's Degree in related field preferred.
- License / Certifications : LSW, LSW Conditional, MHRT-C, or Provisional MHRT / C required.
- Experience : Five years of relevant experience in healthcare required with psychiatric setting preferred.
