Minimum Requirements :
Parent / caretaker with personal experience with behavioral health disorders. Minimum High School Diploma or equivalent, and ideally bilingual.
Certified Peer Specialist certification required.
ESSENTIAL JOB FUNCTIONS :
- Provide outreach and support to all family members enrolled in the CCBHC at every stage, from pre-admission to discharge.
- Work collaboratively with parents / legal guardians in advocating, parenting, care planning and effectively engaging and interacting in a strengths-based manner.
- Provide psychoeducation, skill building; participate in home visit planning and processing.
- Create opportunities for families to support each other via group meetings and activities.
- Provide warm hand-off referrals to community-based support services.
- Actively participate as an integral part of the CCBHC multidisciplinary team including family care planning and collaboration.
- Perform the basic accountability tasks related to the job, which include compiling and maintaining all necessary records.
- Assist administrative staff in collection of information for program effectiveness.
- Actively engage in clinical supervision.
- Respond in a positive and respectful manner to diversity.
- Participate in all trainings for the Whole Family Health Model.
- And other duties as assigned
ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES :
- Knowledgeable in peer-based practice.
- Able to connect with people living with mental illness, and / or substance abuse.
- Knowledge about mental health, physical wellness and stress management and local resources that promote health and wellness.
- Be a welcoming, guiding and empowering representative of the program.
- Model a strength-based, family-driven approach.
- Attend trainings to continually increase competencies.
- Demonstrated commitment of the value of diversity, equity and inclusion.
