Job Overview :

The Mental Health Worker will work with adult consumers in the community who suffer from Severe and Persistent Mental Illness.

Mental Health Worker provides comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning that incorporates cultural appropriateness.

Qualifications : Required :

Required :

  • Bachelors Degree in mental health field
  • 1 year of relevant, post degree experience
  • Valid NJ Driver's License required with no restrictions

Preferred :

  • CPR / First / AED preferred, will be provided if certificate is not active.
  • Bilingual Spanish or ASL preferred, but not required

Scheduling Requirements :

  • 7 : 00 am 3 : 00 pm
  • Monday to Friday
  • Full Time

Essential Functions :

  • Fulfills case management services which include coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, establishing community supports, supportive counseling, crisis intervention, and providing transportation as needed
  • Mental Health Worker will be responsible for the assessment of consumers throughout enrollment in program
  • Mental Health Worker will collaborate with family and health care professionals to develop Individualized Rehabilitation Plans that are consumer centered
  • Mental Health Worker will also works with administrative staff to ensure that all services are documented based on billing and regulatory standards
  • The mission of the department is to assist consumers in their recovery and keep them stable in the community while working towards consumer s ability to have stable independence
  • The focus of this program is to institute a psychiatric rehabilitation model and work on goals to assist in independent living
  • The mission of the department is to assist consumers in their recovery and keep them stable in the community and aid in the transition to further independent living
  • Candidates will need to be patient and possess strong motivational skills to support the consumers in their individualized goals
