About the Job : PURPOSE STATEMENT :
PURPOSE STATEMENT :
The Counselor provides treatment and support to assist patients in their recovery from addictions, such as alcoholism, drug addiction, eating disorders, or other behavioral problems, assists with modifying problem behaviors;
may also provide counseling services to families of clients.
Roles and Responsibilities :
Hours : 1 : 00pm-9 : 00pm
EDUCATION / EXPERIENCE :
- Requires Bachelor's degree in social or health services field
- Degree must be from an accredited college or university.
LICENSES / DESIGNATIONS / CERTIFICATIONS :
- Licensure or certification for counseling required (CADAC, LCSW, LSW)
- CPR certification and de-escalation training required (training available upon hire and offered by facility).
- First aid may be required based on state or facility.
ESSENTIAL FUNCTIONS :
- Participates in patient treatment planning and continuum of care.
- Conducts group and / or family sessions, under supervision, as scheduled and documents as required.
- Co-facilitates assigned group or family sessions and assists with modifying problem behaviors.
- Responsible for initial assessments, as well as follow up assessments for patients.
- Prepares individual treatment plan for each assigned patient.
- Ensures all documentation regarding patient care, treatment and incidents is completed timely and in a clear, concise manner.
- Acts as a liaison between referral sources and patients.
- Provides crisis intervention to patients, as needed.
- Plans for aftercare for assigned patients.
- Acts as a liaison with insurance carriers, as requested.
- Provides case management duties for patients, ensuring individualized quality care.
- Evaluates the need of the patient and determines if referrals to other programs or facilities is needed.
- Provides patient and family education relative to substance use disorder / psychiatric problems.
- Conducts educational workshops both within the Hospital and in the community as required.
- Serves as a liaison with self-help groups and EAP professionals.
- Conducts ongoing assessments of chemical dependence services required by the Hospital treatment programs.
- Assists in the training of new clinical staff and interns, providing direction and clinical guidance.
