Job Description
The program manager will lead and oversee program teams that support individuals with disabilities within the Twin Cities metro area.
This role ensures high-quality service delivery, financial sustainability, compliance with state regulations, and training and supporting program teams to create and implement personalized plans that help individuals achieve their goals.
Qualifications :
Candidates must meet the qualifications for a Designated Manager as per 245D requirements, with at least three years of supervisory experience in a program providing direct support services to individuals with disabilities or those aged 65 and older.
Additionally, candidates must meet one of the following educational and experience criteria :
- Bachelor's degree in a human services-related field and one year of full-time experience providing direct care services to individuals with disabilities or those aged 65 and older.
- Associate degree in a human services-related field and two years of full-time experience providing direct care services to individuals with disabilities or those aged 65 and older.
- Diploma in a human services-related field from an accredited postsecondary institution and three years of full-time experience providing direct care services to individuals with disabilities or those aged 65 and older.
- At least 50 hours of education and training related to human services and disabilities, and four years of full-time experience providing direct care services to individuals with disabilities or those aged 65 and older, under the supervision of a qualified staff person.
Key Responsibilities :
Personnel Management :
- Support the professional growth and development of program staff through effective performance management, regular communication, feedback, training, and development.
- Participate in recruitment, professional training, performance evaluation, and development opportunities for direct service staff.
- Ensure programs are adequately staffed and maintained within budget.
- Maintain high-quality documentation and ensure appropriate quality assurance measures are in place.
- Facilitate communication between the management team and program staff.
Client Services Coordination :
- Screen referrals, coordinate tours, and develop service contracts based on the needs of supported individuals.
- Conduct client intake, admission, and assessments, ensuring individualized service plans are developed, implemented, reviewed, and revised as necessary.
- Attend client support team meetings as needed.
- Ensure high-quality documentation of individualized service plans as required by licensing standards.
- Investigate reports of possible neglect or abuse of vulnerable adults, identify incident patterns, and implement corrective actions.
- Assist in developing daily client programming and serve as a liaison between guardians and other service providers.
- Participate in on-call rotation.
Program Development :
- Ensure programs align with the organization’s mission and promote a positive work environment.
- Develop quality contracts, proposals, reports, and presentations as needed.
- Keep programs, policies, and procedures updated with state rules and legislative changes.
- Build and maintain strong community relationships and identify key areas for new program development in response to community needs.
- Conduct program evaluations and quality assurance reviews.
- Participate in continuous quality improvement initiatives.
- Assume the responsibilities of the Designated Coordinator when necessary.
Skills and Abilities :
- Strong leadership skills in fostering a culture of accountability and responsive communication.
- Effective time-management, organizational, and prioritization skills, with the ability to make sound decisions using good judgment.
- Experience supporting vulnerable individuals and excellent problem-solving skills, including negotiation and conflict resolution.
Working Conditions :
- Frequent use of standard office equipment (telephone, computer, printer, and copy machine).
- Prolonged periods of sitting and / or standing.
- Occasional lifting up to 50 pounds.
- Frequent full range of motion, manual dexterity, and eye-hand coordination.
- Driving between residences throughout the Twin Cities area as needed.
