Job Description :
PURPOSE OF JOB : A non-exempt clinical position responsible for performing a variety of duties in an out-patient setting where you will be assisting providers with patient care and managing clerical, environmental, and organizational tasks.
Serves as a liaison to patients, insurance companies, pharmacies and other members of the healthcare team.

III. JOB CRITERIA AND PERFORMANCE STANDARDS :
Fulfills patient care responsibilities as assigned
Assists providers with managing patient flow
Performs patient triage and obtains vital signs
Reviews patient's personal, family and social history
Reconciles medications
Prepares the exam room and assists with minor procedures
Assisting patients as needed with walking, dressing, etc.
Documentation in electronic medical record
May be responsible for performing :
Phlebotomy
Injections and Immunizations
EKGs
Depression Screening
Falls Risk Assessment
Pulse Oximetry
Audiometry
Visual Screening Test
Laboratory testing-including but not limited to :
Rapid Flu
Rapid Strep
Rapid Mono
Rapid UA
Rapid Hemoglobin A1c
Rapid PT / INR
Rapid CMP and BMP
Rapid COVID test
Urine Drug Screening
Fulfills clerical responsibilities as assigned
Telephone Triage
Medication and testing prior authorizations
May be responsible for :
Referrals
Outreach to patients to close care gaps for cancer screenings, medication adherence and diabetes care
Outreach and education to patients that have been recently seen in the Emergency Room
Outreach and education to patients that were recently discharged from the hospital
Obtaining medical records from patient's healthcare team and other healthcare facilities
Processing medical record requests
Completing forms as directed by the provider
Fulfills environmental responsibilities as assigned
Setting up instruments and equipment according to department protocols
Cleaning exam / procedure rooms, instruments, and equipment between patient visits to maintain infection control
May need to use autoclave to disinfect instruments
Manages inventory of medical supplies, drugs and vaccinations
Fulfills organizational responsibilities as assigned
Respecting / promoting patient rights
Responsible and accountable for patient experience
Escalating patients concerns when appropriate
Responding appropriately to emergency codes
Promoting a culture of positivity and teamwork
Care Navigation
Coordinates care with patient's healthcare team
Knowledge of Value Based Payment Programs

Job Qualifications : QUALIFICATIONS :
QUALIFICATIONS :
Education : High school diploma, Medical Assistant diploma preferred
Experience : Minimum one year of recent experience working in a medical facility in a clinical role preferred
Other Skills : Must possess a current CPR certification and current health records with the appropriate immunizations to work in the health care field.
Knowledge :
Knowledge of health care field and medical office protocols / procedures.

Ability to plan, prioritize, and complete delegated tasks.
