Position Summary : The Discharge / Reassessment Nurse at Benevolent Family Services LLC is responsible for managing member's admission and discharge processes.
This includes conducting assessments, developing care plans, coordinating with interdisciplinary teams, ensuring all documentation is completed accurately, and providing support to members and their families throughout the transition process.
Key Responsibilities :
Reassessments :
- Conduct reassessments of members currently in services to determine their continued healthcare needs and eligibility for services.
- Implement updated individualized care plans based on reassessment findings.
- Coordinate with essential team members to ensure members remain on track with service goals.
- Communicate with members, families, and referral sources to gather necessary information and provide education about available services.
- Complete all readmission documentation accurately and promptly.
- Coordinate care transitions between different levels of care and services.
- Serve as a liaison between members, families, and healthcare providers.
Discharges :
- Assess member's readiness for discharge and develop discharge plans in collaboration with the interdisciplinary team.
- Coordinate discharge planning meetings with members, families, and healthcare providers.
- Ensure all discharge documentation is completed accurately and timely.
- Provide education and support to members and families regarding post-discharge care and resources.
- Follow up with members post-discharge to assess their adjustment and address any concerns.
Documentation and Compliance :
- Ensure all documentation is completed in compliance with agency policies, state regulations, and Medicaid requirements.
- Report any incomplete or missing paperwork that impacts care or compliance of services.
- Participate in quality assurance and compliance activities as required.
Member Support :
- Provide emotional support to members and families during readmission and discharge processes.
- Address any member or family concerns and escalate issues as necessary.
Qualifications :
Education : Associate or Bachelor's degree in Nursing (ADN or BSN) from an accredited nursing program.
Current and valid Registered Nurse, QMHP, or LMHP in the state of Virginia.
Experience : Minimum of 2 years of nursing experience, preferably in a community health, mental health, or similar setting.
Skills :
Strong assessment and clinical skills. Excellent communication and interpersonal skills.
Ability to work effectively in a multidisciplinary team.
Proficient in electronic health records (EHR) and documentation.
Knowledge of Medicaid regulations and requirements.
Strong organizational and time-management skills.
Ability to travel to member homes and community settings as needed.
Work is performed in a clinical office setting. Work may travel to members of the community. May require evening and weekend work.
