- This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to : medication order approval and distribution;
- assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens;
coordination of patients' pharmaceutical needs with physicians and nurses.Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.This position is also responsible for supervising Pharm.
D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff).
