We are currently seeking a dedicated Family Practice Nurse Practitioner to join our team.

Job Duties :

• Provide comprehensive primary care services to patients of all ages, focusing on preventive care, health maintenance, and disease management.
• Conduct thorough assessments, diagnose acute and chronic illnesses, and develop individualized treatment plans in collaboration with patients and their families.
• Perform routine health screenings, immunizations, and health education sessions to promote wellness and disease prevention within the community.
• Manage and coordinate patient care, including referrals to specialists, follow-up appointments, and medication management.
• Maintain accurate and up-to-date medical records, ensuring compliance with regulatory standards and best practices.
• Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated and comprehensive care to patients.
• Participate in quality improvement initiatives, clinical research projects, and continuing education opportunities to enhance clinical skills and knowledge.
• Embrace our community-focused approach to healthcare delivery, fostering strong relationships with patients, families, and community stakeholders.

Qualifications :

• Master’s degree in Nursing from an accredited program.
• Current certification as a Family Nurse Practitioner (FNP) and license to practice in Utah.
• Strong clinical skills with proficiency in conducting physical examinations, interpreting diagnostic tests, and managing common primary care conditions.
• Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
• Ability to thrive in a collaborative team environment, while also demonstrating autonomy and initiative in clinical decision-making.
