We are currently seeking a dedicated Family Practice Nurse Practitioner to join our team.
Job Duties :
- Provide comprehensive primary care services to patients of all ages, focusing on preventive care, health maintenance, and disease management.
- Conduct thorough assessments, diagnose acute and chronic illnesses, and develop individualized treatment plans in collaboration with patients and their families.
- Perform routine health screenings, immunizations, and health education sessions to promote wellness and disease prevention within the community.
- Manage and coordinate patient care, including referrals to specialists, follow-up appointments, and medication management.
- Maintain accurate and up-to-date medical records, ensuring compliance with regulatory standards and best practices.
- Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated and comprehensive care to patients.
- Participate in quality improvement initiatives, clinical research projects, and continuing education opportunities to enhance clinical skills and knowledge.
- Embrace our community-focused approach to healthcare delivery, fostering strong relationships with patients, families, and community stakeholders.
Qualifications :
- Master’s degree in Nursing from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP) and license to practice in Utah.
- Strong clinical skills with proficiency in conducting physical examinations, interpreting diagnostic tests, and managing common primary care conditions.
- Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
- Ability to thrive in a collaborative team environment, while also demonstrating autonomy and initiative in clinical decision-making.
