Job Summary : Program Coordinators are responsible for the direct supervision of Mental Health Rehabilitation Professionals and adult individuals with disabilities in various settings such as the home, community and vocational sites.
It is the Program Coordinator's responsibility to see to the specific needs of each individual served, provide and teach opportunities, provide options for informed choice, and follow guidelines according to each individual service and support plan.
Program Coordinators strive to continuously improve practices, maintain the company culture of employee respect and retention and maintain premium quality care, independence and advancement of clients that is the hallmark of the company.
Job Duties :
Direct Support : Program Coordinators provide direct support services to members using person-centered approaches and strategies that support members to be fully engaged and include in each aspect of his or her daily life, have maximum choice and control, and gain independence.
Facilitation of Services : Program Coordinators have knowledge sufficient to fulfill their roles related to individual service plan development, implementation, and review.
Health and Wellness : Program Coordinators promotes the health and wellness of all clients.
Organizational Participation : Program Coordinators are familiar with the organizational mission, vision and values.
Documentation : Program Coordinators are aware of the requirement for documentation in his or her organization and is able to manage these requirements efficiently.
Client Empowerment : Program Coordinators enhance the ability of the individual to lead a self-determining life by providing the support and information necessary to build self-esteem, and assertiveness and to make decisions.
Assessment : Program Coordinators are knowledgeable about formal and informal assessment practices in order to respond to the needs, desires, and interest of the individuals.
Advocacy : Program Coordinators should be knowledgeable about the diverse challenges facing individuals (i.e. human rights / client rights).
Community and Service Networking : Program Coordinators are knowledgeable about the formal and informal supports available in his or her community and are skilled in assisting the individual to identify and gain access to such supports.
Building and Maintaining Friendships and Relationships : Program Coordinators support the participant in the development of friendships and other relationships.
Communication : Program Coordinators are knowledgeable about the range of effective communication strategies and skills necessary to establish a collaborative relationship with the individual.
Crisis Intervention : Program Coordinators are knowledgeable about crisis prevention, intervention, and resolution techniques and should match such techniques to particular circumstances and individuals.
Professionalism : Program Coordinators pursue knowledge and information necessary to perform job duties.
Vocational, Educational, and Career Support : Program Coordinators are knowledgeable about the career and education related concerns of individuals.
Staff supervision, training, and development : Program Coordinators coordinate and lead competency-based direct support staff training and professional development activities, including coaching and mentoring.
On Call : Program Coordinators are expected to always be on-call during the week. Program Coordinators are expected to be on-call on the weekends, unless they are off duty as a result of being part of an on-call coverage rotation with another Program Coordinator.
It is the responsibility of the Program Coordinator to communicate properly to their staff the on-call rotation.
