We are seeking a motivated and experienced Lead Caregiver Patient Care Aide to join our dedicated team. The ideal candidate will have a passion for providing top-notch care to individuals in need, ensuring their well-being, and fostering a positive environment.
As a Lead Caregiver or CNA at My Happy Place Homecare, you will be responsible for covering Spartanburg, Greenwood, Greenville, or Greer, South Carolina.
Responsibilities :
- Initiate care for new clients efficiently, conducting initial assessments and developing personalized care plans.
- Provide hands-on patient care, including assistance with daily activities such as bathing, grooming, dressing, and mobility.
- Lead and supervise a team of caregivers, ensuring adherence to care plans and the delivery of high-quality services.
- Maintain a clean and secure environment for patients, strictly adhering to infection control protocols.
- Provide emotional support to patients and their families, fostering a caring and compassionate atmosphere.
- Communicate effectively with healthcare professionals, family members, and other team members.
- Collaborate with fellow healthcare professionals to ensure comprehensive and holistic patient care.
- Meet clients for the first time, assess their needs, and develop personalized care plans to support their Activities of Daily Living (ADL).
- Provide direct in-home caregiving, assisting with personal care, medication management, mobility, and other ADLs as required.
- Mentor and guide other caregivers, providing support and guidance to enhance their skills and capabilities.
- Collaborate with the care team and other stakeholders to ensure effective communication and coordination of care.
- Must be available for a weekend on-call rotation.
- Will have on-call scheduling responsibilities
- Lead Caregivers or Lead CNAs responsibilities will consist of being available to cover any call out during the hours of 7 AM-7 PM and start new clients when needed.
