Job Summary :
Guardian Home Care is seeking compassionate and dedicated individuals to join our team as Family Caregivers through Medicaid’s CFI (Choices for Independence) program.
If you have a loved one who qualifies for Medicaid’s CFI program, you may be eligible to become their paid caregiver. As a Family Caregiver, you will provide essential, high-quality care to your family member, ensuring they can continue to live comfortably at home.

Key Responsibilities :
- Assist with personal care and hygiene (bathing, dressing, grooming)
- Provide mobility support (help with walking, transferring, or using mobility aids)
- Prepare meals and assist with feeding if needed
- Remind your family member to take medications as directed
- Perform light housekeeping tasks (laundry, cleaning, organizing)
- Provide companionship and emotional support
- Document daily activities and report any changes in condition
Qualifications :
- Must be a family member of a Medicaid / CFI-eligible individual
- Must complete paid caregiver training provided by Guardian Home Care of NH
- Compassionate, patient, and committed to providing excellent care
- Reliable and able to follow a care plan
- Good communication and time management skills
- Ability to lift or assist with mobility support (as needed)
