Under general supervision and in accordance with the policies, procedures and guidelines established within the organization, unit and the Nurse Practice Act, the RN manages the care delivery for patients in the hospital throughout the length of stay to achieve quality outcomes and timely discharge.

Employees providing direct patient care must demonstrate competencies specific to the population served.

Bachelor of Science in Nursing degree from a four-year college or university and 2 years clinical experience required.

Licensed Registered Nurse in good standing with the State of Michigan

BLS required within six months after hire

Must obtain nationally recognized certification in wound and ostomy management, within 2(strongly suggested) years of taking the position and then maintain certification.

CWOCN and Continence certification preferred.

Complete continuing education programs relevant to areas of practice.

  • Must be able to operate and perform basic computer programs associated with documentation and record keeping in the department.

Work which produces high levels of mental / visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time.

Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.

General Summary

The WOCN (wound, ostomy, continence nurse) Clinical Nurse Coordinator coordinates and provides consultative and direct clinical care services for ostomies, fistulae, tubes, dermal wounds, and come continence care throughout inpatient and outpatient areas, in both adult and pediatric (BMH specific) populations.

Provides education services for nursing, medical and allied health staff, and consumers in regards to areas of expertise.

Works in close collaboration with providers to establish patient treatment goals and plan of care, as well as monitor outcomes to adjust plan of care.

Develops education materials, policies, procedures, in-servicing for staff development, and recommendations for wound / ostomy equipment for the system included under the umbrella of the WOCN training and expertise.

Participates with nursing quality and management to develop evidence based practice initiatives in regards to pressure ulcer prevention and management.

Patient Care and Consultation :

Serves as primary educator and resource for patient undergoing ostomy or continent diversion surgery, through the continuum of care.

Assesses patient / family peri-operatively to devise and modify a care plan for the promotion of physical, psychological, self-care and social adaptation.

Upon consult), is responsible for preoperative selection of stoma site based on the patient’s anatomy, activity level, and lifestyle.

  • Educates and counsels the patient and family peri-operatively.
  • Initiates appropriate referrals based on evaluation of patient’s physical, psychological, and self-care status, identifying medical, surgical intervention or discharge needs.
  • Provides long-term follow up for ostomy patients through outpatient clinic

Provides consultation to staff regarding management of patients with established ostomy or continent diversion.

  • Assesses patient in regards to self-care status and routines, knowledge level regarding ostomy / continent diversion, and problems related to ostomy management or rehabilitation.
  • Develops care plan to address care needs relative to ostomy or continent diversion.
  • Assists patient and / or staff with problem solving as indicated.

Provides consultation and assistance to staff in management of patients with draining wounds and fistulas.

  • Assesses patients to determine type and volume of output, peri-fistular skin status, abdominal contours, and activity level.
  • Devises plan of care for containment of drainage and odor and protection of perifistular skin.
  • Educates staff / caregivers regarding wound management and provides assistance as indicated.

Serves as consultant in the management of patients with urinary and fecal incontinence.

  • Assesses patient to determine causative and contributing factors for incontinence and patient / caregiver management goals.
  • Initiates referrals when indicated for further workup or medical / surgical intervention.
  • Develops care plan for continence care(e.g. bowel or bladder training programs, instruction in pelvic muscle exercises, recommendations regarding appropriate containment products and skin care)
  • Educates patient and staff / caregiver regarding measures for support of continence.

Provides consultation and assistance to staff in the management of patients with potential or actual alteration in skin integrity.

  • Assesses patient to determine etiologic (risk) factors for skin breakdown.
  • Provides guidance to staff in implementation of protocols to control or eliminate (risk) factor, including selection of appropriate support surface.
  • Develops comprehensive wound management plan and assists staff in wound care when indicated.
  • Evaluates patient response to treatment and modifies treatment plan as indicated.
  • Educates patient and staff / caregivers regarding wound management plan.
  • Establishes protocols / guidelines for appropriate and cost-effective utilization of therapeutic support surfaces
  • With MD order, provides conservative sharp debridement of avascular tissue.
  • With MD order, utilizes AgNO3 to cauterize closed non-proliferative wound edges, hypertrophic granulation of tissue, and / or small bleeders.
  • Assures comprehensive management of patients receiving Negative Pressure Wound Therapy, to include recommendations for initiation and discontinuation of therapy, dressing changes when designated by provider, and assisting staff with troubleshooting.

Educational Consultant and Mentor :

Orients staff to the WOC nurse role and the consultative procedure. Provides formal in-services on a routine basis and as needed to ensure current evidence based knowledge and practice.

Develops and revises policies, procedures, and guidelines related to wound, ostomy, and continence care according to Evidence-Based Practice.

Provides educational programs or mentoring for healthcare providers including nurses, providers, and students.

Serves as coordinator of own practice and as leader for nursing staff :

Collaborates with financial officers to establish and maintain a current charging system for WOC services. Analyzes stock items and recommends appropriate additions and deletions to assure quality and cost-effectiveness of products used for wound, ostomy, and continence care.

Conducts product evaluations when indicated and submits reports and recommendations based on results. Contributes to research studies related to wound, ostomy, and continence care when feasible.

Serves as agency-wide committees and participates in agency-wide quality improvement projects. Attends professional offerings, to further enhance clinical skills and stay current on clinical standards related to wound, ostomy, continence and skin care.

Participates and in research related to WOC nurse practice including data collection, audits and interpretation of data.
