JOB DESCRIPTION
TITLE : VICTIM SUPPORT COORDINATOR OLDER ADULT PROTECTIVE SERVICES
DEFINITION : Under the supervision of the Victim Support Program Supervisor, the Victim Support Coordinator (VSC) is responsible for developing and effectuating care plans designed to remove imminent risk to identified clients age 60 and above who are victims of abuse, neglect, financial exploitation, or abandonment.
NOTE : This position is dependent on funding from a VOCA grant. Position will not be offered to any candidate until funding is finalized.
This is a Grant Funded position that is scheduled to run through October 1, 2020. This date may change depending on future funding of this grant
QUALIFICATIONS : Education :
Education :
BA / BS / BSW or MA / MS / MSW in Social Work or related social science
Experience :
3 years direct service experience. Familiarity with protective services, victim services, crisis intervention, mental health or case management with the elderly
is preferred. Ability to develop and effectuate care plans for challenging cases. Second language abilities preferred.

Additional Requirements :
- A report of criminal history record information from the state or Federal Government.
- A valid driver's license, a good driving record and access to a fully insured car and, when necessary, transporting of clients.
- Availability via cell phone at all times during scheduled working hours, including lunch hours, meetings, etc.
- Completion of Pennsylvania Department of Aging's Older Adult Protective Services training as set forth in the Regulations.
- Signing the Pennsylvania Department of Aging's Confidentiality Statement.

DUTIES AND FUNCTIONS :
- Receives cases after OAPS investigation is complete to develop a care plan to assist victims and remove imminent risk.
- Upon receiving substantiated case, make referrals to appropriate agencies and services as indicated.
- Assess the need for emergency intervention including petitioning for involuntary mental health commitments, protection from abuse orders, emergency intervention and access orders, when appropriate.
Testifies in court as needed.
- Accompanies and assists police in delivery and enforcement of above-mentioned court orders.
- Provides on going case management to victims who are older adults in need of protective services as defined by the Older Adult Protective Services law reassessing as required.
- Documents all case activity in SAMS and the ETO system in accordance with State regulations, agency policies, and best practice.
- Establishes and maintains relationships with public service providers, including mental health and health care providers, police district offices, and other community agencies.
- Coordinates case planning with other service providers and other service delivery systems.
- Arranges and conducts case conferences with others involved in care plans
j. Locates and / or mobilizes resources to meet victim needs.
- Participates in on going problem solving, program evaluation and development with all staff.
- Participates in orientation and training, in service training, and attends regular staff meetings.
- Uses regular conferences with supervisor to evaluate work, improve job performance, and enhance professional growth.
- Assists with outreach and public education.
- Identifies resource and service gaps, which inhibit the provision of high quality care to victims of abuse, neglect and financial exploitation.
- Works with supervisor in reporting program results to appropriate persons.
- Performs other related duties as assigned.
